Both the University and Morehead Associates, the survey administration consultants, are fully committed to the confidentiality of the 2006 Staff Opinion Survey.
From the instant you log onto the Morehead Associates web site and the survey is electronically submitted, no one but Morehead Associates will ever see it again. You only respond on Morehead’s web site. The Morehead Associates website is encrypted (https) for additional confidentiality.
Morehead has committed to creating reports only when at least five employees have responded. This "rule of five" prevents anyone from knowing who made what response. None of the individual survey responses are available to UCSF.
Demographic information is reported at the overall organization level only. If you are uncomfortable answering one or more of the demographic questions, they are not required. You can leave them blank.
